  •   Care Assistant (Medlock Court)

    Hours:

    Job Purpose:
    This is an exciting opportunity to join our Residential Enablement Service at Medlock Court to take a Care Assistant role, providing a range of personal, social, emotional and practical care tasks for the wellbeing of service users and their carers. You will share our values and have a passion for providing the highest quality care, enabling service users to exercise control by maximising their independence.

    Key Responsibilities:

    • To ensure quality of service to service users by ensuring person-centred plans are followed and personal needs are met.
    • To take directions from lead care staff and be part of a multi-disciplinary team.

  •   Resource Manager (Supported Living)

    Hours:
    36.66 hours per week

    Salary:
    £30,507 - £34,788

    Location:
    Based at Ena Hughes Resource Centre, 2 Ellesmere Street, Failsworth, M35 9AD (though borough-wide travel is necessary and therefore the successful applicant must have a full driving licence and a car available for use, unless disability requires you to use alternative transport)

    Job Purpose:
    To oversee and manage the day to day delivery of high-quality care and support services within the organisations Supported Living schemes promoting respect, equality and independence and providing an environment where people can thrive and become the best they can be.

    Key Responsibilities:

    • To carry out the role of Resource Manager you will need a good understanding of the Care Quality Commission (CQC) regulatory framework to ensure the service is compliant and experience of working with vulnerable people.
    • You will be responsible for the on-site delivery of care and facilities management of the building.
    • You will provide leadership to the on-site team ensuring that staff work in a person-centred way and in partnership with service users, their families and carers ensuring their voice is heard, making a positive difference for the people we support.
    • To ensure the service delivers high quality care, achieves the aspirations of service users and delivers the outcomes required by all stakeholders.

    Job Purpose:
    We are looking for enthusiastic and skilful individuals with bags of energy to join our successful and growing Supported Living Service.

    Key Responsibilities:

    • To provide support to people with toileting, washing, dressing, eating, personal hygiene, mobility according to their need.
    • To be supported to / administer medication as directed by GP or consultant, to ensure safe storage and keep accurate records in line with the medication policy and in accordance with agreed care plans, and liaise with the appropriate medical professionals as necessary.
    • To assist and encourage individual development with regard to domestic and social and recreational skills. In order to achieve set goals and to have involvement in the Person Centred Planning. To maintain records of patterns of care and observations.
    • To work a range of shifts on a 24/7 basis including evenings, weekends and Bank Holidays as required in accordance with service user needs and to undertake these as required.

    Job Purpose:
    We are looking for an enthusiastic and skilful person with bags of energy to join our successful and growing Shared Lives service.

    You will have a passion for providing the highest quality standards of care and be a proactive practitioner with the ability to work well within an established tight-knit team.

    The purpose of the role is to assist the Registered Manager in the delivery of a Shared Lives service for vulnerable adults with varying needs. Under the guidance of the Shared Lives Manager the post holder will be expected to work alongside commissioners to identify service users’ individual needs in relation to their appropriateness for placement within the Shared Lives service. This will involve a comprehensive review and matching process specific to the role of a Care Coordinator.  You will also have a responsibility to be involved in the recruitment, training and approval of Shared Lives carers and the ongoing monitoring of both service users and carers within the service.

    The focus of this role will be to work with those young people who have learning disabilities and / or other additional needs who are likely to transition to adult's services in the next 12-36 months to assess their suitability to become part of the Shared Lives scheme in Oldham.

    This is an exciting opportunity to join our Extra Care Service to take a lead role in providing a range of personal, social, emotional and practical care tasks for the wellbeing of service users.     Key Responsibilities:

    • To ensure that all duties directed by the Registered Manager and detailed in the service user plan and risk assessments are carried out in full, providing high quality personal care to the client. 
    • To work supportively and pro-actively alongside GP’s, nurses, physiotherapists and any other professionals involved in the care and wellbeing of the client.
    • Maintain awareness of the role in providing assistance, guidance and support to service users, allowing them to remain as independent as possible.
    • Demonstrate a commitment to equality and diversity and a respect for the rights of service users to privacy, dignity and confidentiality.
    • To support the management team in the effective and efficient running of the service and its development.
    • To update and review care plans regularly and to participate in the completion of risk assessments and care plans to ensure the needs of service users are met.

  •   Care Worker (Extra Care Housing)

    Job Purpose:
    Care Workers are employed to work with clients to provide care and support as detailed specifically in the service user plan provided for each client. All clients are individuals who will have a person-centred, holistic assessment of their needs which forms the basis of their service user plan. The role of the home care worker is to meet their clients’ needs and to ensure they are supported as detailed in their service user plan ensuring at all times that their privacy, dignity and independence are maintained. This will include not only assisting with the client’s personal care needs but also their social welfare. The role may include prompting or administering medication, shopping, cooking and will include light housekeeping duties.

    Key Responsibilities:

    • To ensure that all duties directed by the Registered Manager and detailed in the service user plan and risk assessments are carried out in full, providing high quality personal care to the client.
    • To inform the Manager immediately of any emergency situations, or if the client experiences a noticeable change in their behaviour or care needs.
    • To work supportively and pro-actively alongside GP’s, nurses, physiotherapists and any other professionals involved in the care and wellbeing of the client.
