  •   Reablement Support Workers

    Hours:
    25 hours per week

    Salary Grade:
    Starting at £19,819 plus unsocial banding payment – pro rata

    Location:
    Whitney Court, Southlink Business Park, Oldham, OL4 1DB

    Job Purpose:

    To contribute to a team approach in providing an enablement service 7 days a week, aimed at promoting independence and helping to restore/maintain the customer’s ability to remain within their own home.

    Provide a service that meets the requirements of CQC, departmental policies, procedures and guidelines. 

    Key Responsibilities:

    • To assist with the monitoring, implementation and evaluation of care plans and enablement programmes
    • To complete written records as required with the customer, carer and family following discussions regarding agreed goal planning
    • To provide flexible, personal and practical support (inclusive of personal care needs) which is respectful of customer’s privacy and dignity and in line with their assessed need
    • Maintain accurate records, contribute to reviews/assessments and team meetings and liaise with other agencies and professionals such as social workers, nurses and therapists when required.

    Job Purpose:
    This is an exciting opportunity to join our residential enablement service to take a lead role in providing a range of personal, social, emotional and practical care tasks for the wellbeing of service users and their carers. You will share our values and have a passion for providing the highest quality care, enabling service users to exercise control by maximising their independence. 

    Key Responsibilities:

    • To ensure quality of service to service users, by offering support and guidance to colleagues and other professionals through liaising with service users, their carers,  Health and Social care staff, Voluntary and other agencies
    • To provide appropriate levels of support and mentoring to colleagues
    • To work independently and as part of a team to ensure effective service delivery whilst taking responsibility for the service in the absence of the manager, liaising with and seeking guidance from the On Call Manager

    Job description:

    To provide a supportive and enabling environment for vulnerable people with mental health problems, challenging behaviour, autism, ABI (Acquired Brain Injury), learning difficulties, in a rehabilitation setting and individual tenancies in accordance with assessed identified plans of care.

    To assist the management team ensuring the expectations of Commissioners, the Care Quality Commission is met through compliance with Policies and procedures and good professional standards. To alert and report any issues or cause for concern to line manager. To provide an environment where both service users and staff can thrive and become the best they can be.

  •   Care Workers - Extra Care Housing

    Job Purpose:
    Care Workers are employed to work with clients to provide care and support as detailed specifically in the service user plan provided for each client. All clients are individuals who will have received a person-centred holistic assessment of their needs which forms the basis of their service user  plan. The role of the home care worker is to meet their clients’ needs and to ensure they are supported as detailed in their service user plan ensuring at all times that their privacy, dignity and independence are maintained. This will include not only assisting with the client’s personal care needs but also their social welfare. The role may include prompting or administering medication, shopping, cooking and will include light housekeeping duties. Care workers must work as directed by the Manager and detailed in the individual service user plan

    Key Responsibilities:

    1. To ensure that all duties directed by the Registered Manager and detailed in the service user  Plan are carried out in full, providing high quality personal care to the client.
    2. To inform the Manager immediately in regards to any emergency situations, or if the client experiences a noticeable change in their behaviour or care needs
    3. Proactively support GP’s, nurses, physiotherapists and any other professionals involved in the care and wellbeing of the client.

  •   ICES Assistant Coordinator (secondment)

    Job Purpose:
    A secondment opportunity has arisen within Oldham Care and Support for MioCare Group staff or those working within an Oldham Cares partner organisation (including Oldham Council, Oldham CCG, Pennine Care NHS Foundation Trust and Northern Care Alliance). The Integrated Community Equipment Service (ICES) is accessed by a range of health and social care professionals within Oldham and provides equipment to adults and children to support with moving and handling, bathing, mobility, etc. 

    We are looking to recruit a part-time, ICES Assistant Coordinator on a fixed term contract to backfill some of the roles and responsibilities for the ICES Coordinator whilst they are undertaking a project. Working under the supervision of the ICES Coordinator and with the administrative support of the ICES Business Support Officer, you will contribute to the continuous development of the service, ensuring a high standard of service is maintained by our service provider and that resources are used effectively.
