  •   Reablement Support Workers

    Hours:
    25 hours per week

    Salary Grade:
    Starting at £19,819 plus unsocial banding payment – pro rata

    Job Purpose:
    This is an exciting opportunity to join our growing enablement service supporting individuals whilst promoting their independence, and helping to restore/maintain their ability to remain within the community. You will have excellent communication and organisational skills, be able to prioritise your workload and work well within a team. You will share our values and have a passion for providing the highest quality care and support, respecting individual’s dignity, choice and preferences. You will need to hold a full driving licence for this role and have access to a vehicle.

    Key Responsibilities:

    • To assist with the monitoring, implementation and evaluation of care plans and enablement programmes
    • To complete written records as required with the customer, carer and family following discussions regarding agreed goal planning
    • To provide flexible, personal and practical support (inclusive of personal care needs) which is respectful of customer’s privacy and dignity and in line with their assessed need
    • Maintain accurate records, contribute to reviews/assessments and team meetings and liaise with other agencies and professionals such as social workers, nurses and therapists when required
    • Ensure the appropriate use, maintenance and storage of equipment supplied for the customer’s enablement programme.  As part of the monitoring process alert a senior staff member if change of equipment or equipment repair is required.

  •   Care Workers - Extra Care Housing

    Job Purpose:
    Care Workers are employed to work with clients to provide care and support as detailed specifically in the service user plan provided for each client. All clients are individuals who will have received a person-centred holistic assessment of their needs which forms the basis of their service user  plan. The role of the home care worker is to meet their clients’ needs and to ensure they are supported as detailed in their service user plan ensuring at all times that their privacy, dignity and independence are maintained. This will include not only assisting with the client’s personal care needs but also their social welfare. The role may include prompting or administering medication, shopping, cooking and will include light housekeeping duties. Care workers must work as directed by the Manager and detailed in the individual service user plan

    Key Responsibilities:

    1. To ensure that all duties directed by the Registered Manager and detailed in the service user  Plan are carried out in full, providing high quality personal care to the client.
    2. To inform the Manager immediately in regards to any emergency situations, or if the client experiences a noticeable change in their behaviour or care needs
    3. Proactively support GP’s, nurses, physiotherapists and any other professionals involved in the care and wellbeing of the client.

  •   Assessment and Reviewing Officer

    Location:

    Job Purpose:
    To support managers to deliver a high quality enablement service, that meets  customer’s needs.  To work with customers, carers and other professionals in completing assessments, reviewing, monitoring and recording progress on enablement packages. To assist the management team in ensuring the expectations of Commissioners and the Care Quality Commission is met through compliance with policies and procedures and good professional standards. To alert and report any issues or cause for concern to the line manager. To provide an environment where both service users and staff can thrive.

    Key Responsibilities:

    1. Physical Care: Assisting individuals with toileting, washing, dressing, personal hygiene and mobility according to their needs. To support with medication as directed by GP or consultant. To ensure safe storage and keep accurate records in line with the medication policy and in accordance with agreed customer plans, and liaise with the appropriate medical professionals as necessary. To undertake medication audits / checks in accordance with procedure and report any discrepancies or concerns to manager and Pharmacy ensuring the safety of the person at all times.
    2. Individual Development: Assist and encourage individual development with regard to domestic and social skills in order to attain set goals and to record outcomes and achievements towards independence. To complete assessment of risks and monitor and update individual service user plans whilst maintaining accurate records for audit and data collection purposes.
    3. Recording Skills: To monitor and review relevant paperwork pertinent to individual customers i.e. Medication Mar Charts, Service User Plans, Finance Logs, etc. To maintain accurate and detailed written supervision records and complete direct observations with support staff. Be responsible for recording all customer information on mosaic from initial referral, step up/down and purchasing of services. Update continually on each customer including transfer to post enablement process via needs assessment .
