  •   Reablement Support Worker (Reablement Team)

    Hours:
    25 hours per week

    Salary:
    £19,819 (pro-rata) with an additional 10% payment for unsocial banding 

    Location:
    Based at Whitney Court, Southlink, Oldham, OL4 1DB

    Job Purpose:
    This is an exciting opportunity to join Reablement Team supporting individuals whilst promoting their independence and helping to restore / maintain their ability to remain within the community. You will have excellent communication and organisational skills, be able to prioritise your workload and work well within a team. You will share our values and have a passion for providing the highest quality care and support, respecting individual’s dignity, choice and preferences. You will need to hold a full driving licence for this role and have access to a vehicle.

    Key Responsibilities:

    • To assist with the monitoring, implementation and evaluation of care plans and enablement programmes.
    • To complete written records as required with the customer, carer and family following discussions regarding agreed goal planning.
    • To provide flexible, personal and practical support (inclusive of personal care needs) which is respectful of customer’s privacy and dignity and in line with their assessed need.
    • Maintain accurate records, contribute to reviews / assessments and team meetings and liaise with other agencies and professionals such as social workers, nurses and therapists when required.
    • Ensure the appropriate use, maintenance and storage of equipment supplied for the customer’s enablement programme.  As part of the monitoring process alert a senior staff member if change of equipment or equipment repair is required.

  •   Senior Community Support Worker (Supported Living)

    Hours:
    Various - 30 and 35 hours per week positions available

    Salary:

    Location:
    Based at Ena Hughes Resource Centre, 2 Ellesmere Street, Failsworth, M35 9AD (though borough-wide travel is necessary and therefore the successful applicant must have a full driving licence and a car available for use, unless disability requires you to use alternative transport)

    Job Purpose:
    To provide a supportive and enabling environment for vulnerable people with mental health problems, challenging behaviour, autism, ABI (Acquired Brain Injury) and / or learning difficulties in a rehabilitation setting and individual tenancies in accordance with assessed identified plans of care.

    To assist the management team ensuring the expectations of Commissioners and the Care Quality Commission is met through compliance with policies and procedures and good professional standards. To alert and report any issues or cause for concern to line manager. To provide an environment where both service users and staff can thrive and become the best they can be.

    Key Responsibilities:

    • To assist the manager in the daily operational management of the establishments, staff and resources. To compile rotas in line with: allocations, current care plans, individualised budgets, CHC funding. To anticipate and react to changing needs by adjusting allocations accordingly ensuring best value and most appropriate use of resource. To report any changes in need to Finance / Line Manager.
    • To provide supervision, guidance, direction to staff, including the induction of new members to the organisation.
    • To assist and encourage individual development with regards to domestic and social and recreational skills, in order to attain set goals and to record outcomes and achievements towards independence.
    • To maintain detailed written records of patterns of care and observations of service users. To take lead role in monitoring and reviewing relevant paperwork pertinent to individual service users.
    • To assist in matters of household management which include cleaning, shopping, menu planning, cooking, budgeting, laundry and safety / security issues.

    Key Responsibilities:

    • To provide support to people with toileting, washing, dressing, eating, personal hygiene, mobility according to their need.
    • To be supported to / administer medication as directed by GP or consultant, to ensure safe storage and keep accurate records in line with the medication policy and in accordance with agreed care plans, and liaise with the appropriate medical professionals as necessary.
    • To assist and encourage individual development with regard to domestic and social and recreational skills. In order to achieve set goals and to have involvement in the Person Centred Planning. To maintain records of patterns of care and observations.
    • To work a range of shifts on a 24/7 basis including evenings, weekends and Bank Holidays as required in accordance with service user needs and to undertake these as required.
    • To play an active role in maintaining and promoting the scheme, acting as an ambassador and role model at every opportunity.

    Job Purpose:
    This is an exciting opportunity to join our Extra Care Service to take a lead role in providing a range of personal, social, emotional and practical care tasks for the wellbeing of service users. You will share our values and have a passion for providing the highest quality care, enabling service users to exercise control by maximising their independence.

    Key Responsibilities:

    • To ensure that all duties directed by the Registered Manager and detailed in the service user plan and risk assessments are carried out in full, providing high quality personal care to the client. 
    • To work supportively and pro-actively alongside GP’s, nurses, physiotherapists and any other professionals involved in the care and wellbeing of the client.
    • Maintain awareness of the role in providing assistance, guidance and support to service users, allowing them to remain as independent as possible.
    • Demonstrate a commitment to equality and diversity and a respect for the rights of service users to privacy, dignity and confidentiality.
    • To support the management team in the effective and efficient running of the service and its development.
    • To update and review care plans regularly and to participate in the completion of risk assessments and care plans to ensure the needs of service users are met.

  •   Care Worker (Extra Care Housing)

    Job Purpose:
    Care Workers are employed to work with clients to provide care and support as detailed specifically in the service user plan provided for each client. All clients are individuals who will have a person-centred, holistic assessment of their needs which forms the basis of their service user plan. The role of the home care worker is to meet their clients’ needs and to ensure they are supported as detailed in their service user plan ensuring at all times that their privacy, dignity and independence are maintained. This will include not only assisting with the client’s personal care needs but also their social welfare. The role may include prompting or administering medication, shopping, cooking and will include light housekeeping duties.

    Key Responsibilities:

    • To ensure that all duties directed by the Registered Manager and detailed in the service user plan and risk assessments are carried out in full, providing high quality personal care to the client.
    • To inform the Manager immediately of any emergency situations, or if the client experiences a noticeable change in their behaviour or care needs.
    • To work supportively and pro-actively alongside GP’s, nurses, physiotherapists and any other professionals involved in the care and wellbeing of the client.
