  •   Assessment and Reviewing Officer

    Hours:
    30 hours,  permanent

    Salary Grade:
    £22,401 (pro rata) subject to pay & review 2017-18

    Location:
    Whitney Court, Oldham 

    To support the Enablement Managers to deliver a high quality Enablement service, that meets customer’s needs. To work with customers, carers and other professionals in completing assessments, reviewing, monitoring and recording progress on enablement packages.

    To assist the management team in ensuring the expectations of Commissioners and the Care Quality Commission is met through compliance with policies and procedures and good professional standards. To alert and report any issue or cause for concern to the line manager. To provide an environment that both service users and staff can thrive.

    Key Responsibilities:

    • Assisting individuals with toileting, washing, dressing, personal hygiene and mobility according to their needs.

    • To actively support equality & diversity in all areas of work as appropriate.

    • To support with medication as directed by GP or consultant. To ensure safe storage and keep accurate records in line with the medication policy. To undertake medication audits / checks.

    • To monitor and review relevant paperwork pertinent to individual customers. Be responsible for recording all customer information on mosaic from initial referral, step up/down and purchasing of services.

    • To liaise with carers, other professionals, internal and external agencies local community and promote positive communication ensuring best outcomes for those we support.

  •   Community Support Worker - Waking Night Staff

    Hours:

    Job Purpose: 

    To provide a safe and responsive environment in the late evening and early morning for vulnerable individuals with complex health needs in accordance with the services policies and good professional standards. To provide a range of personal, social, emotional and practical care tasks for the wellbeing of service users and their carers.

    Key Responsibilities

    1. To have a knowledge and understanding of the needs of vulnerable adults including those with a Learning Disability, Autism, mental health and Acquired Brain.
    2. To support and enable/re enable service users who may require assistance with personal care and physical tasks such as administering medication
    3. To ensure the care, health and wellbeing of each individual is provided, whilst reporting and recording concerns to relevant people and liaising with colleagues, medical professionals and any other appropriate body
    4. To maintain appropriate records and pass on any relevant information about the needs of the service user to your manager or other designated persons and ensure compliance with access to Data and Freedom of information Act.

  •   Home Care Worker - Extra Care Housing

    Hours:

    £8.35 per hour

    Job Purpose:
    Home Care Workers are employed to work with clients to provide care and support as detailed specifically in the service user plan provided for each client. All clients are individuals who will have received a person-centred holistic assessment of their needs which forms the basis of their service user  plan. The role of the home care worker is to meet their clients’ needs and to ensure they are supported as detailed in their service user plan ensuring at all times that their privacy, dignity and independence are maintained. This will include not only assisting with the client’s personal care needs but also their social welfare. The role may include prompting or administering medication, shopping, cooking and will include light housekeeping duties. Care workers must work as directed by the Manager and detailed in the individual service user plan

    Key Responsibilities:

    1. To ensure that all duties directed by the Registered Manager and detailed in the service user  Plan are carried out in full, providing high quality personal care to the client.
    2. To inform the Manager immediately in regards to any emergency situations, or if the client experiences a noticeable change in their behaviour or care needs
    3. To work supportively and pro-actively alongside GP’s, nurses, physiotherapists and any other professionals involved in the care and wellbeing of the client.
